Regional Care Association
We want you to be well looked after at home. Therefore, the University Medical Center Freiburg works in conjunction with outpatient centers such as the PflegeNetz GmbH, and ambulant maintenance services of the Breisgau-Hochschwarzwald and Emmendingen regions. In addition, the University Medical Center has founded the Regional Pflegeverbund (Regional Care Association). This network provides a smooth transition from care in the hospital to outpatient care at home.
The aim of the Regional Care Association at the University Medical Center Freiburg is to facilitate your return home and to optimise conditions for your further recovery. For us, transition of care from the hospital to the home means providing personal and individual consultation as well as high quality care based on coordinated concepts and established standards.
